When to avoid traction

Traction is recommended for most back pain except in the acute phase of an injury. When any area of the spine is inflamed, initially it is important to avoid bilateral traction. This refers to the movement of both sides of the pelvis away from the lumbar spine at the same time. The reason for this is that, during the initial phases of injury, the spinal muscles naturally contract in order to protect the spine from further movement and, therefore, further injury. If traction is applied during this acute phase of muscle spasm, the affected area may feel good while in traction, because the compression is relieved. But as soon as the traction is released, the muscles will tighten, this time with a vengeance, creating more pain than previously. We must always respect the body’s wisdom, but especially during this acute phase of the injury. Therefore the only kind of traction appropriate at this stage is unilateral traction in a neutral position. This safe technique is described below.
